Please review all steps to ensure your

appointment request can be approved.

Step 2: We will review your inquiry form and match you with a counselor in your SimplePractice Client Portal that best fits your needs.

Step 3: You'll receive an email with next steps and a link to your SimplePractice Client Portal.

Step 4: Complete all of the required online intake paperwork in your portal and request a session. In the portal, you can see your provider's availability for the next 60 days.

 

Please note: The required credit card form must be complete to reserve an appointment time. Appointment requests cannot be approved otherwise. We require a card on file for all clients for any copays or other financial responsibility required by their specific insurance plan, for self-pay sessions, and for potential late cancellation fees as laid out in the intake paperwork. We encourage clients to contact their insurance company directly to determine mental health benefits and claim coverage.

Step 5: Attend your first appointment via telehealth in the SimplePractice Client Portal or in-person at our office: 2194 Eastridge Center in Eau Claire, off of Hastings Way, in the same building as Highland Fitness. We are on the first floor, in the back of the building.

Step 6: As a small practice with no front desk or receptionist, you can message your provider directly in your SimplePractice Client Portal with questions about your appointments or care. If you ever need to cancel, you can do so right in the portal (also available as an app!). To avoid the late cancellation fee, cancel your session at least 24 hours in advance.
